This option is available throughout the year and allows for 100% of the proceeds to go directly to our PTA Programs ~ Arts in Education programs, Rec Nights, 8th Grade Ceremony and more! PTA Membership Form - we encourage all parents and teachers to join the Middle School PTA! All PTA board members (Program Chairs) must be a member of our PTA Unit. The PTA is a member organization, make your vote count by joining today!
School Directory Form - form should be filled out and returned to the main office by start of school year. You only need to fill out a form one time when your child enters in 6th grade or if you have a change of any information. If you no longer would like your information published in the directory then you need to state so in writing prior to print deadlines. The directory will be sent electronically to families home email address.
The District uses the "School News Notifer" (SNN) system to help keep parents and residents more in touch with key happenings in the school district and it has expanded its offerings. SNN is an opt-in system where individuals can sign up to receive e-mail alerts from the district administration when specific things occur, such as emergency school closings or delays, sex offender registry news or events & activities specific to the Middle School. Go to the SNN homepage and create your profile and select which of the 8 alerts you would like to receive via email and/or cell phone via a text message. Once you select your alerts and click "subscribe," you will be all set! (You can change your selected alerts at any time.)
Forms for PTA Program Chairs
Check Request Form to be reimbursed for purchases made for a PTA event - this must accompany the receipt, and authorized by the committee chairperson. Keep in mind we are tax exempt - tax paid on any purchase will not be reimbursed. Contact Lorraine O'Donnell, Treasurer, for reimbursement.
Deposit Verification Form, deposits should be given to the treasurer Lorraine O'Donnell within 24 hours of receipts - if you cannot, then give to an Officer. Fill out this form and attach the deposit receipt. Write the committee name and your name on the deposit receipt and return it to the treasurer. All deposits must be dual counted - no exceptions.
